you will not be able to keep nocturne off of either you in lane or your hypercarry at any stage of the game. his spellshield + fear will guarantee he gets on you if he's not stupid enough to get it baited out, and after that he'll just run you down and kill you. don't play ivern into this.
Katarina
you just absolutely dumpster her. Katarina is the textbook lane to pick ivern in to. She can do nothing to you, she's too squishy to eat your bushwack harrass, she's free food for your jungler, and landing E & Q against her is a breeze because she has to jump on you to hurt you. Your ult interrupts her and will run her down and help you dive her. she won't be able to waveclear. she won't be able to poke you out or put damage on you. she won't be able to all-in you until the point where she's probably already been shut down. her only hope is to hit 6 and roam or to gank you, but you should follow her or coordinate with your jungler to countergank.
Morgana
she will push you in to tower, and you are not good at waveclearing and do not want to waste your bush CDs trying to lasthit at tower. lategame, she'll invalidate your Q. morg is rare to see mid, but she is THE passive laner, which isn't good for you. Mid Ivern wants facerollers who int into him, not people who sit back and waveclear all lane.
Vel'Koz
velkoz will push you into tower and poke you out of lane. he'll keep you from using your bushes or your Q and melt through your moderate tankiness. he'll scale up and be a monster late-game while you lag behind. bad bad lane. if you end up having to play this, your best bet is to gank him repeatedly and just keep him from ever playing the lane, because you can beat him in the first few levels with your OP shield and your Q is a jungler's wet dream. If VK is smart, though, he won't let you get that close. the good news is that most people play VK bot, not mid, so you shouldn't see him here.
Yasuo
yasuo can't really trade favorably with you in lane, and most yasuos will underestimate your power and int into you. however, he will outscale you if he survives the laning phase. If every yasuo, instead of just most yasuos, played aggressive against you, this lane would have a rating of 1. The yasuos who sit back and don't fight you and wait for plays are the reason this lane is a 2.
Ziggs
similar to morgana, ziggs will play outside your range, have you pushed to tower and will blast it down in no time. however, his spells will be easier for you to dodge than, say, those of velkoz.
Lux
don't get hit by her binding, or land yours, and you should win this lane. if she tries to use her spells to clear the waves, just play up and force her away. block her damage with your E and walk at her. hit her with autos if she goes up to cs. She outscales you somewhat, but you beat her in lane, as in most matchups.
Zed
zed will have trouble against you pre-6 and post-6, as long as you play around your cooldowns and don't get ganked. He can waveclear, but if he's using his abilities on the wave, you can just fight him. After a certain point in the game, he'll be able to kill you with his combo in teamfights every time. But hey, that means he's not using it on your carry. before that, make sure he isn't landing free poke on you and make sure to land your Q if he goes in. He'll probably coordinate with his jungler to kill you at 3 or 6 (or after), which will be a prime opportunity for a counter-gank.
Anivia
she has no kill threat on you, but after 6 she'll just push every wave to tower and farm for free, which isn't fun for you and will make her very fat. hope she plays up too far and coordinate with your jungler to gank her out, because you won't be able to roam against her waveclear.
Caitlyn
considering the range she plays at, you probably won't land Q on her unless you're significantly better than her, and her traps will keep you from using your bushes. she'll be a monster you can't handle later in the game. hope your team has someone else who can deal with her.
Hecarim
hecarim will hate your guts if you're any good at landing your Q. He might be forced to ult early. he might be forced to juke it and lose his window. Maybe it will just land and he'll be sad. don't sit there and facetank his Q for too long, but do feel free to punish him with your autos as you walk away from him.
LeBlanc
you're immobile and low-range, so you might think she would be hard for you, but you actually push and cs better than she does, your E keeps her from bursting you, and your Q is often easy to land against her and keeps her from backing out. make sure to abuse your autos against her, because she's very squishy and can't take too many. the real threat from leblanc comes from her roam, but rejoice! you outscale her :-) she can still out-trade you if she lands her E and doesn't take bushwack damage, so make sure to use your autos and try not to let her burst through your shield totally for free. after level 6, don't let her full-combo you and try to wait for your ult or your jungler to fight her.
Quinn
A quinn you lane against is low-threat. a quinn in toplane is a major threat. she will dodge your Q as she goes in, jump on you, and kill you when she ganks. if you wait for her to jump on you to Q her, she's already landed her damage and her blind will keep you from following up. after 6, she won't even be in lane for you to roam on her and gank her. she's very different in your own lane, and much more fightable.
Maokai
ivern can't poke out or trade as effectively with tanks like maokai in lane, which is one of the reasons i don't actually think he's viable in the toplane. However, maokai is played mid sometimes, and you may end up seeing him. maokai clears waves reasonably well, and a good one will just flash-W you and kill you with his jungler if you try to fight him. He's tankier than you, and does surprising damage.
Master Yi
Assuming he's jungler: land your Q (that includes timing it with his Q), and don't int into him, and you should make life hard for him at every stage in the game. Don't try to fight him 1v1 if he has ult up (nobody wins that). Assuming he's your lane opponent: lol enjoy your free win
Nasus
As a general opponent: nasus is a great gank for you, as at level 6 you can just throw your golem at him, dive him, and kill him with your toplaner. Don't try to fight him 1v1 while his ult is up, even if yours is too. later on in the game, you can Q him and watch him sit there and fume, but it might be better to save it for someone else.
Garen
kite back, use your Q against his Q and your E against his E.. a bad garen will eat harass for days trying to fight you or overextend for farm early and never get on you. a competent garen, which is what you'll see if you don't play in the baby leagues, will flash Q you and kill you when it counts. As i've said before, ivern doesn't do well against champs who can sustain through his harass while they farm and then rush him down, which is why i don't recommend you take him top.
Gangplank
if gp wastes mana using his Q on you, you'll win lane early. if you're good at killing his barrels, you'll win lane early. If you manage to outplay gp early and shut him down, it's your game to win. if gp doesn't bother with any of that, farms up, waits for trinity force or crit items or whatever he's building and just walks at you, cleanses your Q, and fights you, he'll win. If he goes even, he will outscale you and you will never be able to fight him again. this is another lane that you dumpster early and lose late. wait for your ult if you want to take long trades against him.
Ornn
this is why you don't play ivern top.
Rengar
you know why. But it's not just the W synergy that works well. Rengar can turn your E into an enemy bursting machine, and he can quickly snowball a game out of control, which is exactly what you want. the two of you can also root a target for a long time.
Amumu
you help him get in and can set up a counter-gank, but there are better synergies. that's my opinion. i don't normally list "low" synergies, but i think many people rate the two higher than they deserve together.
Garen
your shield is pretty good with his E, your Q helps him get in, and you help him stay alive.
Galio
decent- to great synergy, depending how smart the galio is. He can make some silly plays with his justice punch in combination with your Q by angling it backwards, and of course his taunt will do wonders.
Nasus
decent synergy. Ivern helps with a lot of nasus's major issues.
Graves
Your E and Q help him surprise enemies, stick to targets, and gank/countergank. You two are great together at every stage of the game, and you have the double synergy of him being both a bruiser and a marksman.
Gragas
ivern has some synergy with most melee junglers. gragas in particular will be happy not to have to use E-flash to land his ult combo.
Jax
ivern+jax is godlike. the two of you work so well together.
Udyr
ivern+udyr is very strong, particularly when counter-ganking, but udyr himself is still a flawed champ that can struggle to function later in the game regardless of who he's paired with.
Lee Sin
lee's a great early-game champ, which is most of the actual reason this pairing is strong, and he works great with your E and decently well with your Q.
Master Yi
yi is a lategame hypercarry, but you love him more than he loves you. most of the synergy here is your ability to help him set up a counter-gank. Later on in the game, he doesn't need your Q to engage and probably won't be concerned with the same targets, but he'll still like your shield.
Sett
Hell yeah this is good. why wouldn't it be good?
Fiora
you and fiora work decently well together when you both have the same goals, though she may prefer to flank and try to get on the enemy backline, which is not where you're going to be.
Fiddlesticks
surprise! fiddles's ult likes your Q and your E, and he ALSO likes your W if the two of you are clever. he's a notorious counter-ganker, and his fear will help the two of you lock down certain targets for even longer. however, everything the two of you do together requires setup to execute, and in a game where things don't always work out the way you plan them, you can easily end up getting nothing accomplished.
Jayce
jayce doesn't benefit so much from your shield, as he usually only goes in to hammer form to *finish* kills, but you may help keep people off of him later on or lock a target down long enough for him to burst them.
Mordekaiser
this is pretty good, i guess.
Olaf
this is strong for the reasons you'd think.
Kennen
these two actually have better synergy than ivern+amumu. Your Q gives kennen that extra little boost of movement that helps him get on to the enemy team in a fight, and your E will keep him up long enough to do his job.
Kayn
these two like each other all right.
Nunu & Willump
what nunu really synergizes with is your bushes, but he likes your Q too. the two of you are occasionally awkward, but can do some real good work, kind of like with fiddlesticks.
Skarner
this is a disgusting combo. Skarner doesn't even need to suppress the target you're Q-ing, because there's usually someone else nearby he wants more.
Karthus
you and karthus just can't seem to get along. There are few junglers that ivern just doesn't synergize with at all, but this happens to be one of them.
Yasuo
there are champions with more reliable knockups, and he doesn't really care about your Q, but your E could be helpful for him.
Singed
another disgusting combo, similar in many ways to skarner+ivern.
