I use Flash mainly because of its defensive qualities, i.e.: hit Ambush then Flash through a wall, getting out of range of AoE spells, dodging an ambush, etc. Survivability in an ambush or bad team fight is crucial. There's nothing wrong with living to fight another day. However it's also a great offensive spell. Tower-dives, pursuing near-dead enemies, or instant repositioning in a team fight.
I chose Exhaust because it slows and reduces damage on the target. It allows you to briefly pursue or escape a single opponent. You can also 1v1 a squishy physical opponent during early game to get that First Blood!
As you can see, there are no defensive items here, with the arguable exception of Frozen Mallet because of the HP boost. I rely on superior firepower to outmatch my opponents. However, there are some circumstances which might require defensive items, so don't hesitate to adjust according to the armor/magic qualities of the enemy team.
As far as skill rotation goes:
Expunge
If you're low on mana, or don't need to keep up with your target, don't use Venom Cask and just go straight to Expunge. When ganking, you probably won't need Spray and Pray either. But this is late in the game when you've become unstoppable.
Twitch is a weak champion, so try to take mid then sneak up top to help. Otherwise go straight to the speed-boost, hit Ambush, and rush the top. Harass opponents until one is vulnerable. The best thing you can do is get in a few shots and wait before you use Expunge so Deadly Venom has time to tick off some HP. Once you hit level 6, start using Spray and Pray on your opponents to force them to retreat.
Mid game:
Between level 8 - 12, you might have a few kills already, and you probably have Infinity Edge and Zeal. You must constantly be on the go, neutralize or capture points, stay with your team during fights, meanwhile killing minions to bolster your cash flow.
Late game:
After you reach level 18, you should have a decent amount of kills and you're probably finished with your 4th item (including boots). Continue to kill minions between team fights
In a team fight, you will want to position yourself as far away from the melee as you can, while also in range of the squishy targets you intend to annihilate. In other words, you will want to be to the side of the melee. If you must flee, you will probably run sideways from battle instead of backwards. If you end up moving around in the battle, pay attention to location effects you might run through. The last thing you need is to take unnecessary damage, encouraging your opponents to switch their attention to you. Also remember, Spray and Pray affects multiple targets, so use Venom Cask, as needed, for some security while you're moving around. Once you have 1 or more targets at very low HP, pop Expunge to take them out.
I cannot stress the importance of patience and positioning in a team fight. It will make a huge difference between knocking over targets and getting yourself knocked out early in the fight. Wait for your targets to use their stuns, snares, or silences then make your move.
It is very easy with this build to get double and triple-kills, with some extra attention to detail it is not out of the ordinary to get quad-kills and penta-kills. But don't expect it, those are generally due to luck and good team work than skill.
