QSS should be a mandatory for everyone but tanks and bruisers/supports that are built tanky (since they will likely have the resists to be able to sit there and play damage sponge while CC'd) unless the enemy team has a serious lack of CC.
Tryn, for example, is vulnerable to CC. A common strategy to bring a Tryn down is to Exhaust him, CC him after he ults, and tag him with Ignite right as his ult ends (since the typical Tryn won't heal until AFTER the ult ends and a high level Ignite will easily power through the half-strength heal he'll get.) With a QSS, you can do any number of things to stop this from happening. You can break the Exhaust, allowing you to cause damage normally. You can break the CC during your ult, giving you time to escape (or continue causing mayhem.)
This applies to pretty much any hero who is squishy and does not have a baked-in Cleanse like Gangplank. Best thing is that QSS isn't especially expensive, so it's not too hard to put one into your build that game without drastically altering your normal item build.
It's a hugely underrated item.

Quicksilver Sash when there are TWO or more of the following spells on the enemy team:
Children of the Grave
Hemoplague
Nether Grasp
Diplomatic Immunity
Infinite Duress
Soul Shackles
Idol of Durand
There may be a few more, but I can't think of them at the moment.

It's less than 1500 gold for an item that provides a large chunk of MR and also gives you a Cleanse. You should buy one at any time you feel the enemy team has enough CC to warrant having one. If you find yourself eating CC chains, you should buy a QSS.

Then again, AP Casters usually take Zhonya's Hourglass for survival, Tanks/Trollers (to the other team) take GA, Tryn takes Undying Rage (Lol) while the rest just runs and bait the 1st 3 groups :P
