You need to learn how to carry. You can be sub-par player in terms of mechanics, and you can still have 1600 ELO because you know how to carry your team. I carried even with support, I'm @ 1180 ELO atm (dodged down to 1100 from 1350, because I was duoqueueing a lot and didn't feel confident in carrying by myself.).
How many games have you played? People tell me your ELO should be pretty accurate after around 100 games played in ranked.
Just keep trying. If you are good you will pull yourself out of it. If you are bad but are critical of yourself and watch for errors you will get better then pull yourself out of it.

I've played over 3000. Almost 1000 in ranked (combined loss and wins) and around 2000 for normal games.

Solo Q at lower elos isn't about winning the lane.
It's about destroying it.
You gotta obliterate your lane opponnent so hard that you can then roam mid or even bot, claim drag, destroy people in teamfights.
Winning the lane at that level isn't enough. If you're better than they are, you should be able to do it.

I don't go for destroying turrets because that leaves them with the opportunity to free farm. That's bad against late-game champs like Irelia. I always try to keep both turrets up at top lane as long as possible (Unless it really needs to go down i.e in a losing game and teammfights are necessary). I try to harass as much as possible to zone them, I stack minions on one side my side of the map to deny last hits, etc. Destroying the lane would imply pushing the turrets hardcore right? I just can't fathom how that can be any good for myself.

True...but I rarely find that to work very well. In ranked games, people don't surrender when they are behind by a lot because they know they have the possibility to win the game in with one solid push after winning a team fight. I've been there and done it so I know it is possible. Maintaining turret control while roaming is harder than it looks especially if the other lanes are pushed.

Currently, my ELO is 1146 and frankly I feel it is not where I should be. I try as hard as I can to do my job in a team game, but I can never win the game. Most of the time, I blame this on my team mates. Yes, I'm aware that almost everyone in the low ELO range has this mentality, but I honestly believe I am one of the few that are prime examples of being on a bad team. When I started rank, I actually dipped down to the low 900s. It was demoralizing playing in such a low ELO. I gave up and didn't try my best until a few months later. I started winning a lot of games and managed to climb up to 1345...but shortly after I met a few trolls in that ELO range which caused me to go back down to where I am currently at.

Based on that replay, would you guys consider my skill level equivalent to around 1100 ELO?

Update: Managed to get to 1261 ELO from 1159 in two days. Hopefully things will keep getting better.
