You make the right choice in holding off on grabbing Sheen until mid game, seeing as you are maxing Q last, Sheen's synergy with Yorick's kit is delayed and not prevalent early game. Good itemization and buy order choices.
I do not like that you run armor penetration runes / masteries. Most Yorick players actually go for the magic penetration mastery, as a lot of your early game damage is magic They will grab the point in Summoner's Wrath, 3 in Brute Force, 4 in CDR and 1 in magic penetration.
For quints, I would suggest switching to AD or Flat Health Regeneration.
You also keep flat armor and mr per level runes regardless of your lane opponent, something I also saw in your Riven build. I believe against AD you should have flat Armor seals and MR per level glyphs, and against AP you should have Armor per level Seals and flat MR glyphs. This is a simple switch you might want to consider making.
I like that you mention that you can use your ghouls to block skill shots. I like that you mention that your ultimate should be used on your team's AD Carry. However you might also want to include the concept of using it on AP Hyper-Carries if they are in danger, like Ryze and Cassiopeia.
I think you should include more sight ward specifications. Blue team should ward Tri Bush, and enemy wraiths exit. Purple team should ward Blue exit, and the bush of the 2nd jungle exit to the right of Blue teams Blue buff. Yorick has little to no escapes and is VERY vulnerable to ganks.
I like that you list Trundle and Cho'Gath as counters, but you should include Nasus as well. His natural life steal allows him to ignore ghoul damage mostly, and he will get Q stacks off of your ghouls, giving him more Q farm than he could get on his own just free farming. Nasus won't kill you or take your tower, but he will turn into a huge late game threat if you give him free Q stacks by attempting to harass. Champions with natural life steal or healing like Xin Zhao and Irelia also have the possibility of doing well against Yorick, though it is more circumstantial than the outright counters.
A few typos I have noticed in your Introduction... "addiction" should be "addition" (information about Cho'Gath as a counter) and "grand mana" should be "grant mana"
