In my opinion, Nocturne is your literal nightmare, able to spellshield your Q and fear your during your charge-up duration, he'll out-damage you early allowing him to counter jungle you without fail unless your team comes to assist you, ward out your jungle and try to make sure he wont put you behind too much
Vayne
probably the only ADC on this list, your second mortal enemy, her true damage proc which scales with your own max health allows her to do an insane amount of damage to you before you know it, capable of Tumbling out of your Q, she'll give you a hard time, try to lock her down first if you can
Elise
A dangerous opponent indeed, her health percent damage from her Q can shred you down if you lack the Magic Resistance for it, she can out-sustain you in a fight due to her spiderlings, try to kill them as fast as possible to reduce her damage output
Fiddlesticks
A pretty easy matchup if you know what you're doing, pre-6 his fear wont last very long which allows you to just walk up to him and cancel his Drain with a decently charged Q, this allows you to force him to either flee himself or you end up killing him, post-6 you can just ult out of his Fear and kill him, dont let him snowball on you though, his ult will most likely still hurt
Master Yi
Mobile, True Damage, can become Untargetable? thats pretty much all Yi is, the reason why he isnt ranked as high on the Threat Level is because the best way to counter true damage is to stack health, and his weak early game and complete lack of CC allows you to cancel his Meditate with your Q, and if you need to run away from him, not even Yi will be able to catch up on you once you use your ult to escape, try to snatch some camps from him when you can and try not to let him snowball on your team
Sejuani
Since her small rework to her basic abilities she has become quite the powerhouse, with her W on-hit now dealing damage based on your max health instead of her Q, can has a means to knock you out of your Q charge up without having to waste it on her primary source of damage (a good Sejuani wouldnt waste it at that moment anyway but the point still stands), careful of her as her W has a surprisingly low cooldown
Volibear
A strong opponent, his primary damage comes from stacking Health which deals more damage the less current health you have, and his passive turns the tides of fights, try to bait out his Q before wasting your own, your flat damage from your Q is enough to chunk him low, dont over commit to the fight once his passive goes down, it has a pretty long cooldown so try to kill him when its down
Trundle
Trundle is a scary dude in a one on one fight, especially against tanks, pre-6 he'll steal damage from you with his Q, and ramp up his attack speed while slowing you in the process with his pillar, post-6 his ult will cripple you, as he steals your Armor and Magic Resistance, while chopping down your health based on its max percentage, try not to fight him alone as he benefits a lot more from lifesteal which allows him to out-trade you during fights
Amumu
This matchup can go 2 ways and it all depends on whatever Amumu would have maxed at that point, would he have maxed his W he has the possibility of literally running past you letting your health leak like liquid, he has a pretty weak early-game and has a pretty rough time getting back in the game when behind, try to keep your distance, while abusing the range of your Q and the slow of your E, make use of your W to prevent him from dealing too much damage to you over time
Maokai
I dont know a lot about Maokai, only that his Twisted Advance deals damage based on your max health, with is already quite severe against Sion, and with him having a knockup enabling him to knock you out of your Q charge up, play it smart, he doesnt have a lot of damage early, but Maokai can still kill you would he play it smart
Udyr
Probably one of the more annoying junglers to play against, a stun to cancel your Q charge up, attack speed and damage without having to build a lot of it, force him away from you, his bear stance will allow him to run away whenever he wishes, so killing him is almost ruled out, just try to make sure he doesnt put you behind too much
Kha'Zix
With his weak early-game and his terrifying late-game, Kha'Zix can be a tough matchup, try not to fight him alone as he has a lot of damage, but without any hard CC a good Q can squash this bug real good
Wukong
I'd call this an even match-up, as Wukong has damage early-game, but the amount of damage you can take late-game makes up for it, dont forget to use pink wards to see through his stealth
Warwick
With Warwick being one of the most basic junglers in the game, he wont pose much of a threat, but once he gets his Blade of the Ruined King you gotta be careful, as he'll start hurting quite a lot and allows him to out-sustain your damage
Rammus
Hard to catch, harder to kill, what he lacks in raw damage makes up for his utility, try to counter gank him as much as possible as his ganks can turn quite vicious when left unchecked
Shyvana
This is a rather difficult matchup, due to Shyvana's high aggression and her E which lets her basic attacks deals damage based on your max health, her Hybrid damage makes it hard to itemize against her

Items
For items, i like to use Frozen Mallet for its on-hit slow which Sion benefits from, as he he's prone to being kited, especially during his passive, Spirit Visage is useful due to his passive as well, Sunfire Cape is near mandatory on tank junglers to increase their clearspeed, Sion is no expection, Skirmishers Saber is great for tanks due to their ability to take more damage from a target that deals a large amount of damage and the Juggernaut enchantment gives a nice amount of tenacity, while granting health as well, Hydra works great as a sole damage item due to its health regen, active and large amount of AD and the lifesteal is a nice benefit as well, the mobility boots can be switched out for Ninja Tabi later on in the game when the laning phase is over

9/21/0 is a basic Mastery setup for tanks, maintaining a high defense throughout the game while storming in with a decent amount of damage
Runes
Runes
Marks: Attack Damage Marks for increased clear speed, and it provides a small amount of extra damage for ganks, which can come in handy quite a bit
Seals: Armor Seals, these are mandatory and should always be taken
Glyphs: Magic Resist Glyphs, for more overal tankyness, these can be switched in for Health Glyphs if you're going against a team thats more AD
Quints: Health Percent Quints, they will give you an extremely high amount of health in the late-game, as you're health scales infinitely, so will the health granted from these quints

Sion's Infinite Health Stacking
Sion is the only champion in the game who can stack his health infinitely thanks to the passive from his W, this should be taken into account that Sion becomes progressively harder to kill as the game continues to go on, if any lanes have big minion waves headed your way, ask your team if its ok that you clear those instead, as you will make more use of the CS than they will, but in cases where somebody of your team is behind and needs to catch up more than you do, its best to let them take it instead

Sion can activate his W during his other abilities, make use of this to improve your cleartime by activating your W during the charge up of your Q or during your ult to peel more during your initiation and make use of the slow of your E to catch up with fleeing enemies by knocking a minion into them and dont forget that Sion's ult has an extremely low cooldown at rank 3, dont be afraid to run away from enemies with it late-game as its a strong way to make sure your team doesnt get completely aced
