i was watching CLG stream last night and notcied the opppsions malzarhar get health from melefic visions (was like 20,20,20,20) and he had a will of thh anchients, I thought spell vamp didnt work on DoT's ? or did the item buff do something?

Life steal = Auto-attacks.
Spell-Vamp = All ability skills including true-damage, excluding on-hit effects and abilities that work like auto-attacks.
Unless your champion synergizes extra well, Spell Vamp and Life steal should always be second-priority.
Spell-Vamp and Life-steal are instantly shut down if you're stunned.
A good example of theoretically good builds failing is spell vamp Mordekaiser.
Seems like a nice idea, WotA, Gunblade, etc, heal up with your skills and such. But, if you're stunned your build does nothing for your defenses. have below 100 on both stats = Insta-gibbed.
