The only thing that you can do to fix outdated guides is have moderators archive them and PM the author to update it. Outdated guides do nothing good for the website.
But it's still very hard to gain votes/views with a new guide without any help. If you really want your guide to succeed you'll first need it to get noticed by several people so it can get close to the top and gain views on it's own. This means you either ask your friends to vote on it or become active on the forums so people look at your guides.
Also, never make the mistake I did and disable "comment to vote" or you'll get several down votes which will make it much much harder to get it to a decent rating.

Speak for yourself. From what I've seen, most of the guides that deserve to be at the top make it there (though not all top guides are good). Obviously this isn't always true, because there are lots of ignorant people and trollvoters on the internet. But that's what the scout system is for - to help some of the good unrecognized guides rise to the top.
EDIT: I probably made it sound too simple in my original post. Writing a great guide and getting it known does not guarantee its success. I'd say that it would more often than not though.

The outdated guides will lose freshness and will eventually expire, like Madmack's Ashe guide. I don't believe there are any updated guides at the top that are bad - though I only know this for the champions I play.
Not always... The top Maokai guide is extremely outdated. His/Her chapters do not reflect the build in the cheat sheet.
