You completely screw Yasuo at all levels. His windwall is useless and can't get too close if he doesn't want to get a full combo in the face. Just dodge his tornado and it's an easy match. Do not forget to harass with auto attacks to break his shield!
Aatrox
Malzahar suffers from champions with gap-closers. Aatrox's jump isn't exactly a gap-closer but can still complicate your life. Other than this, his E is easy to dodge and is squishy enough to lose his passive without ulting. Play safe
Ahri
It's imperative that you dodge her charm. Try to always have a voidling up, they will save you from her E more than once. Other than this, you ult counters her ult, your E keeps you at a safe distance and, if you land a Q, you screw her up completely. Another easy match
Akali
Shove her to her tower, where she'll be unable to farm properly. Always, and I repeat ALWAYS bring pinks. If you're not fast enough, she'll jump on you and throw her smoke screen before you can combo her. Do not let her roam.
Anivia
Since you have no mobility, her wall can screw you. You both have a great waveclear, though you have it since you start maxing your E, while she starts waveclearing efficiently post lvl-6. Beware of her stun.
Annie
She is one of the few midlaners that does not really suffer the consequences of the elimination of the mana potions. Thanks to her Q, she'll have a free stun each 10 seconds, so be careful. Her E helps to mitigate your voidling's damage, as well as hurt them. You can 100-0 her, and she can do the same with you. Try your best to land your silence if she approaches you. Tough matchup
Azir
He'll poke the hell out of you. You'll rarely get to combo him without losing half HP to his soldiers. Beg for an early gank while his AS is still slow.
Blitzcrank
You might find a mid Blitzcrank. Nothing to worry about, your voidlings will block his Rocket Grab more than once, thus making it harder for him to land his R. Take advantage of the situation once he loses his passive and poke the hell out of him.
Brand
Crazy damage once he lands his W. He has a decent range and can make his ultimate bounce off your voidlings to hurt you even more. This matchup will depend on your skill.
Cassiopeia
Keep your distance, try to farm with your E and move constantly. Do not let her poison you with her Q or W because not only she'll be able to cast infinite Twin Fangs on you, but also you'll be adding stacks to her already OP passive.
Cho'Gath
You'll never kill him, believe me. Even if you get to full combo him, you will have consumed only 3/4 of his health bar. DoT is not effective against him since he sustains like crazy with his E. Try to switch lanes with your top. Otherwise, stay passive and try your best not to die.
Diana
The result of this match will depend on how you move. Post lvl 6 try your best to dodge her Q, or she'll jump on you infinitely to rape you multiple times. Her E mitigates your E's damage. She has the advantage.
Ekko
I have rarely faced an Ekko in the midlane, since he does better in the jungle. You should have the advantage, since he's squishy enough to die to a full combo, making his ultimate pretty much useless. Do not eat unnecesary poke with his timewinder and you'll be fine.
Ezreal
A wild Ezreal appears in the midlane every now and then. He's completely skillshot based, and your voidlings will block his skills more than once, especially if your Malefic Visions were casted on him.
Fiddlesticks
A nightmare as a jungler and as a midlaner. If you're ulting someone, he'll throw an E or a Q at your face. If he's in the midlane, he'll laugh at your Malefic visions as he'll be draining the minions or your voidlings with the W.
Fiora
Don't let her get near. Poke at a safe distance and don't forget to use your auto-attacks to harass, especially early on, when AAs hurt a lot. If she wants to ult, W at your feet, E, and ult her back
Fizz
He's rarely seen now. His trollpole will make your blood boil more than once, and if you kill him, you'll feel a great pleasure, like the one you get when you kill a Teemo. Get the hell away of his shark's range if he gets lvl 6 before you.
Galio
Flat counter. Try not to cry as he heals with your malefic visions, and try not to commit suicide once he reaches 300 MR as he screws your team with his ultimate. If possible, switch lanes. With anyone.
Gangplank
You want to ult me? No problem, I have an orange here. You want to stay in lane? it would be a pity if someone... critted you...
Gragas
No longer seen in midlane. He'll be a little bit tough to kill, but you have the advantage.
Heimerdinger
Your Malefic Visions combined with the voidlings melt Heimer's turrets down. If you don't have mana, kite those turrets! Your auto-attack range is wider than the turrets' AA range, just be careful with the beam. Easy matchup.
Illaoi
I've had the pleasant experience of having a troll in my team saying "mid or feed", forcing me to go top and face this creature. She might seem tough to kill, but actually she needs to get a few items if she doesn't want to die to your full combo. If she lands an E (which you should pray not to happen) remember that if she kills your soul, you get hurt. If you leave the range, she'll slow the hell out of you, leaving you defenseless against any gank and also spawning tentacles everywhere. Having said this, W-E-R- Ignite and ctrl 4 after this to laugh at this BS champion's death
Jarvan IV
If you happen to face a Jarvan IV mid, start crying. He can get next to you thanks to his E-Q combo, and if he ults you, you're dead. He'll be unkillable in late game, so try your best to farm at a safe distance and to not die.
Jayce
You'll rarely see a Jayce mid, especially in low elos. If you do find one, pay attention to his stance. If he's in range stance, try to spawn voidlings, get behind your minions and cast malefic visions on him. He'll have a hard time trying to land an enhanced Q on you because your voidlings will be blocking him. If he's in melee stance, poke and farm at a safe distance, and save your Q for the moment he wants to jump on you. Even matchup, but not impossible.
Karma
I've never faced a Karma in midlane, but I've faced her playing as support and she does crazy damage early on only with the spellthief's edge, imagine in midlane! So my piece of advice for you is to be very careful from levels 1 to 6. She will have the advantage in terms of damage. Post 6, don't be afraid of going all-in on her, she doesn't have anything to stop you, and if you're quick enough, she won't be able to shield herself to mitigate the damage. If you want this to happen, you will have to perform the full combo Q-W-E-R- Ignite. If you don't silence her, she'll shield herself and you won't kill her.
Karthus
Ctrl 4 then Q on him when he starts channeling his ultimate. If by some unknown reason you fall behind or lose the lane (because he farmed better than you or because he got kills with his ult when you were at base) it is obligatory to buy a Zhonya's hourglass, and TIME IT WELL, DO NOT USE ZHONYAS RIGHT AFTER HE STARTS ULTING OR YOU'LL RECEIVE THE DAMAGE ANYWAY. But trust me, you'll win the lane.
Kassadin
BS mobility post lvl 6, , BS magic resistance, BS shield. A well timed Q will interrupt your ult. If you want to win, get as many ganks at early levels as possible. Get a Banshee's veil when facing this matchup, you won't regret it.
Katarina
Push her to her tower, she won't be able to roam or last hit. No roaming + No farming = No items. Kata + no items = free kills for you. If you see her ulting, Q or ult her back. Make that guy regret playing as Katarina.
Kayle
Strange matchup. She'll farm as fast as you, and she also has hp sustain with her W. Abuse her mana problems early on. If you don't aim properly your Q when you do the combo, she'll ult herself and negate approximately 75% of the damage of your combo.
Kennen
Do not even try to land a Q on him, you'll waste your time and mana. Try to farm and dodge his Q, and you'll be fine. As long as you don't feed him, this match won't be as complicated as it seems. And remember that most Kennen players rush Zhonyas as first item, so if you e-w him, and the player is fast enough, he'll use Zhonyas and negate the damage of what should be your full combo. Kog'Maw
He's your puppy, you should not fight. But if you happen to face this dog in midlane, harass him out of the lane, get some boots ASAP so you don't get the true damage of his passive after he dies. With the S6 rework, he has to be immobile to use his W, so it will be easy for you to land your spells.
LeBlanc
Her mobility is somewhat frustrating, and her early burst is insane. If you're going to combo her, use your E first, in case her passive is up, so you can tell which one is the clone and which one is the real one. She'll normally die with an E-W-R- Ignite combo, but remember this tip just in case . Hard matchup
Lee Sin
If by some strange reason you see a Lee Sin in midlane, don't worry, your voidlings will do your job for you. If he happens to land a Q on you, wait half a second and throw a Q under your feet, he won't be able to escape using his W, making it easier for you to do the full combo. Be careful with his early damage, tho.
Lissandra
If you see her casting Glacial Path and you feel that it's going to be to engage, throw a Q to where she's going to appear. Don't stand near minions because her Q does splash damage. remember that her ultimate heals her if she casts it on herself. Other than this, it will be a boring lane. Farm as much as you can.
Lulu
If she knows what she's doing, she'll poke the hell out of you. She does crazy damage, and she'll abuse it early on. Post level 6, you'll have to 100-0 her, or she will ult herself and get away.
Lux
She has more range than you, her AAs will usually hurt more than yours, she'll shield herself when you cast Malefic Visions on her. It doesn't matter if you're full hp, if she snares you, you're dead.
Malzahar
Skill matchup. What makes him counter you, makes you counter him.
Master Yi
If he lands a Q on you, and you're near your turret, get under it, as he'll appear next to you. He's very squishy and his W has a really long cooldown. Abuse this.
Mordekaiser
After his rework, he's not seen in midlane anymore. But if you happen to see one, remember that your Malefic Visions will almost always reduce his shield by 75% approx. Always have vision near the Dragon's pit, and always tell your teammates if he leaves the lane, even if he disappears for one second. Generally, he won't die to your full combo, so try to poke him down instead and force him to go to base.
Morgana
Another pick that's not seen any longer. Get away from his ultimate's range, do not get snared and do not stay foolishly on her W if you're low health. Make her waste her shield and then go ham. If you're not able to make her waste her shield, just farm. This will be a boring lane.
Nidalee
Get some boots early on to dodge her spears, always have a voidling up, and, if by some reason she's stupid enough to get close to you, perform your full combo in her face.
Orianna
Very hard matchup. Once she gets a Chalice of the harmony, she will poke you again and again and again. If you have a jungler with a stun, such as Pantheon, beg for an early gank to get a kill. Don't even try to cast your E on her, as she will shield herself and negate the whole damage.
Riven
Incredible mobility, insane burst post lvl 6 and a ridiculous dash/shield. It is known that Riven is heavily reliant on her spells and that silences screw her up, but the thing is that landing your silence on Riven will be an extremely difficult task. Stay near your turret and farm with E and auto attacks. Don't forget that she can get from her tower to your tower in 3 seconds with her spells, so don't fool around Riven.
Ryze
He's very hard to kill in a 1v1. Your range is superior than his. Try to silence him whenever you can so he doesn't stack his passive.
Swain
Don't get snared and play aggresively pre-6. Get away from his Q's tether, and get a Morellonomicon to counter his healing post level 6.
Syndra
Crazy range, slow, stun, and can harass you each 3 seconds. Use your E to farm and move constantly, as she will abuse you repeatedly if you don't move from side to side to dodge her skills.
Talon
Most Talon players will get their E at level 2 but won't use it before level 6 (unless you walk foolishly near him). Push him to his tower, abuse his weak early damage, buy pinks to counter his ultimate and, if you see him engaging on you, remember that his E teleports him behind you, so instantly throw a Q behind you to silence him. Decent matchup
Teemo
Teemo mid? Ugh. Abuse him early on, he'll have to decide whether to blind you or your voidling, but he'll lose trades anyway. Kill the little satan and buy Oracle's lens post 6 to clean his minefield.
Twisted Fate
Dodge his Gold/Red card. He doesn't really burst you down unless he's extremely fed, but that almost never happens. Farm safely, dodge his wild cards, and interrupt his ultimate chanelling. Nether Grasp seems a better option to it rather than Call of the Void, since the CotV has a one-second delay and you'll generally miss it (trust me)
Veigar
If you get fed, he'll get fed as well. If you don't get fed, he'll get fed anyway. He'll always have more AP than you, it's normal. Most of the times I take Cleanse against Veigars, so I can get away from his Event Horizon. He's a menace to every single AP caster in the game, so be very careful from level 1 to 18.
Vel'Koz
I've only played once against Vel'koz in mid, and it wasn't a big threat. Just be careful that you don't channel your ultimate on his W, as he will knock you up and cancel it. Get boots and avoid his skillshots. Decently hard matchup
Viktor
His laser has a superior range than any of your spells. Pay attention to what spells he's maxing with his nucleus, and once he enhances his W, don't get fooled and ult on his gravitatory field, as it will stun you and cancel your ult. Skill-based matchup.
Vladimir
Damage over time hurts him even if he's in his Pool of Blood. Harass him, he'll probably use his Transfusion to last hit. Remember that he is a late-game champion, so once he builds Spirit Visage and Zhonya's Hourglass forget about killing him
Xerath
Just pray that he's trying a new champion. Otherwise, he'll make your life a living hell. Get boots ASAP.
Zed
Rush Seeker's Armguard and boots. If you see him ulting, Q behind you, then W-E-R-Ignite. Unless he's Faker, it shouldn't be a hard matchup.
Ziggs
Get boots with this little guy and try your best not to get poked down. Don't waste your mana trying to land a Q on him. Ask for ganks early on and also ask your jungler to time their blue buff so you can steal it and deny him 2 minutes of free poke.

I've played around 250 matches with Malzahar and, even though there are many good guides to this incredible character, I wanted to add my own point of view and share my knowledge of the champion with you! I have been put against almost every champion in the game, built him AD, AP, hybrid and he still hurts. So, why playing Malzahar?

Summon Voidling
On every fourth ability use, Malzahar summons a Voidling that remains on the field until it is killed or 21 seconds have passed. If Malzahar dies, the Voidling stays behind but stops attacking.
Voidlings grow in size after 7 seconds, gaining 50% bonus attack damage and armor, and frenzy after 14 seconds, gaining 100% bonus attack speed. It is important to mention that voidlings do not attack just because they want. They prioritize the following targets:
+Targets affected by Nether Grasp +Targets affected by Malefic Visions +Targets attacked by Malzahar
A good use of this passive will help you block most of the damage/CC caused by skillshot-based champions! Make champions like or suffer!
Call of the Void
Malzahar summons two orbs at the target location perpendicular to which way he is facing after a 0.5-second delay, dealing magic damage to all enemies caught between the orbs and silencing them. This pretty nice skillshot is one of the few (and longest) silences in the game. However, and unlike other silences in the game (hello and ) it is hard to land it, and also has casting time, thus giving enemies with high mobility enough time to get away. It is more easy to land this skill in teamfights than in laning phase. Use it wisely. Pro tip: When the game starts, put one point in Call of the Void and cast it three times in base, so that you have a voidling ready to summon in lane.
Null Zone
ACTIVE: Malzahar creates a zone of negative energy for 5 seconds, dealing magic damage every second to all enemies standing on it, capped at 120 against minions and monsters. This is a beautiful skill that you won't be using much in laning phase unless you perform your combo on your enemy, and this is also the last skill you'll want to max. But in late game this skill appears, and then is when the magic occurs.
Null zone deals: 4 / 5 / 6 / 7 / 8% (+ 1% per 100 AP) magical damage of target's maximum health PER SECOND . I will do the maths for you: you find a with 4000 hp, and you have 500 AP.8% of Null zone + 5 coming from your ap means that you will shred 13% of his maximum health per second That is to say, if you manage to get him standing on your W for 4 seconds, you will have dealt 2080 damage (MR apart). Insane.
Malefic VisionsACTIVE: Malzahar infects the target enemy's mind, dealing them magic damage every half second over 4 seconds.
If the target dies during this time, they pass Malefic Visions onto the closest enemy, refreshing the duration and restoring a portion of Malzahar's mana. This is the ability that distinguishes Malzahar. Also known by many flamers as "the ebola" this skill tends to be pretty annoying, because once the ability gets to take 60% of a minion's health, it farms alone. You can go and ward the bushes, the ability will get the cs for you. However, it is important to know when to use it. For example, you put one point in E at level 2 and immediately cast it on a minion. Was it effective? No, because you probably didn't kill that minion, and even if you did, you won't be able to kill the following one. Use your E to farm once you have some points on it, or you'll waste your mana.
Nether Grasp
Malzahar knocks down the target enemy champion and channels for up to 2.5 seconds, suppressing them and dealing them magic damage every half second while the channel holds.
This is your can't-go-wrong skill during laning phase. It is the lockdown to your combo and leaves defenseless every champion (or well, almost every champion...).
If you manage to ult someone in a teamfight, and you're good positioned, it is a safe kill to your team. But if you're mispositioned, you'll end up dying, since you can't move while channeling, and most probably won't kill who you wanted to kill. Remember to not ult champions with or .
This skill has an excellent synergy with since you can cast it while you're ulting! BUUUUUUUUUUUUT↓
IMPORTANT:
If you're going to cast Nether Grasp at full range DO NOT TRY TO IGNITE WHILE ULTING, OR YOU'LL CANCEL IT.
Whereas Nether grasp's range is 700, Ignite's range is 600. Remember this when you're ulting.
