The sustain start is 3x Rejuvenation Bead and 1x Health Potion, but afaik it has only been used on a few top laners under fairly specific conditions. Not something you'd want to be building every game. The beads are basically a pseudo- Doran's Shield when the damage block and hp aren't useful early game. I've seen it run on Shen and Trundle against mages or other matchups that don't have much all-in potential or autoattack harass in the early game.
Doran's Shield is still much more common by comparison, especially against popular top lane picks like Irelia and Gnar.
The rejuv start seems decent, but it's not a good enough reason to build Warmog's Armor. The beads are so cheap that you can easily sell them to buy something stronger with resistances.

Health regen doesn't really have downtime since it's very rare to be at 100% HP except when you're walking back to lane from base. The rejuv start is for sustaining against poke while you farm if you have poor trading in the early game (e.g. Shen or Trundle vs Swain or Gangplank). You're guaranteed to take harass in these lanes.
Aggressive laners also make good use of health regen due to minion aggro, plus they can always force trades to use their sustain, but aggressive laners don't start triple rejuv bead anyway.

Well, in LCK, they did build a bead into Ohmwrecker on Trundle.
I don't see Doran's Shield at all in any games, as it doesn't allow you to start at least with Refillable Potion. Starting with normal potions is a big waste of money and very inefficient compared to Refillable Potion or Corrupting Potion. Also, the health regen of Rejuvenation Bead outshines in every aspect Doran's Shield, that is 14 compared to 6 (or 9 but you get Refillable Potion) and the health regeneration even scales. So with beads you have the way more efficient items through the game than Doran's Shield. Only reason for Doran's Shield would be if you can't survive the first 2 levels without it. Not to mention Ruby Crystal is also superior to Doran's Shield as you can always build something out of it.
