LOOOL :D!!
and @ ShadowReign757 I feel Tiamat would work amazing on him due to the passif! Guess I would have to try it out to consider it a core item.. However tiamat would be more for a laning Nocturne as opposed to a jungle one :)!
@Dante Rebellion:
Getting TriForce first generally feels very unsatisfying. Sheen is a nice starter, but phage and zeal are mid-game type stats that work better with core stats already high. I find it to be a much more satisfying 2nd core item after wriggles and boots, or after boots + 1st core for non-junglers.
If I jungled, it'd probably be something like this:
b.f. sword
-insert a counter survivability item-
The black cleaver would give me the raw damaging power to get some nice kills.
Triforce would fit in right there most games where you start needing to build up survivability.
Lastly,
where's the "whoosh gtfo.." part coming from? o.o
As funny as it seems, Paranoia is only a one-way dash.
@Traa1703:
It would combo quite nicely as a double ganking strat.
Or just group ganking.
TF uses Destiny to see if MIAs are near Noct's target, and give Noct definite vision of his target.
TF spots another person overextended.
Noct uses Paranoia
Double "An enemy has been slain" ensues. :)

All I am going to see is people saying, "RAWR IM A GHOST AND IM GONNA GETCHA!!!!"
But, ANYWHO, I personally think that Twisted Fate is now going to be a first ban/pick when Nocturne comes out. The synergy out of their ultimates causes Twisted Fate to be indirectly buffed. But, I digress.
Item picks:
I would start out with getting a Wriggle's, too. Then, I would get tier 1 boots and [enter attack speed/attack damage item here]. This will buff his overall damage and his passive will get even more ridiculous. Then, get Mercury Treads and a Sheen. So on and so forth...

@mobrebel:
If the enemy picks a TF with a Nocturne, it's simply all about Tanky DPS.
Although they're "nerfing" that next patch.
They're focusing on laning-dominance, so late-game, they'll still be equally annoying.
Noct + TF = squishy team comp.
Build a tanky-dps type, and they're pretty screwed late-game, even if Noct can reduce vision to 300 for a few seconds.
Not saying the combo won't have synergy, but it won't merit any bans.
Nocturne will probably get banned a bunch though. Since, Destiny/Gate isn't quite as troubling as Paranoia.

XD true. But, it also depends on team composition. The team with Nocturne and Twisted Fate could also have 2 tanks and a support/ap. It really depends on the other team as well. Anyway, it is all theory...

One thing worth noting is that Paranoia will force people to group up, so it could get more use than on some other champion.

Ill be jungling with him too.. gonna build him a bit like this:
Charlie
Ditch the Wriggle's and get a
Wriggle's is a powerful jungling item which can help me alot early game..
boots to catch up...
Phage for the slow and health..
Sheen for a bit more damage plus im winning :3 (charlie sheen joke)
Trinity force cause it has a bit of everything which is amazing..
bloodthrister for more AD and lifesteal
PD just a general great item for making you jungle faster, attack faster, chase faster and kill faster
Madred's towards the end cause well... newbs be getting alot of health and this item will shine :D!
Not enough defensive items I know, but I believe this champ is more of a ... whoosh kill, whoosh gtfo.. the only defensive item I may consider will be Banshee's and Quicksilver :P
I laughed out loud at the "whoosh kill, whoosh gtfo" part
And yeah, selling Wriggles for a Bloodrazor at the end seems like a great idea.
