Teams who don't communicate or listen to one another. O__O
First instance - I was in mid against an Ahri, 3/0/0. One gank (not from a jungler), died. Asked them to call ss. Silence. 5 minutes later, another gank. 3 people appear in mid, no help. We lost (this happened to other lanes too) - frustrating because, had we communicated with eachother, we could have easily won that.
Second instance - I ended the game with 14/3/26, and other people were doing just as well if not better - the other team had an average of about 2-3 kills. I and someone else said 'push mid' - nobody pushed mid. We could have won it if that had happened. But then they stacked health and pushed (it was a 5vs4 match - 4 on our part, our Morgana hadn't connected), won after an hours play.

it crashed in the middle of a game changing fight, I came back like 10 min later and we were still winning but not as well...
