The date is probably an error (tons of games have this problem), but you still deserve to be penalized.
Riot is very meticulous about the cases that get punished and pardoned - Support staff go through each pardoned or punished case to give it a final check off.
If you got punished, it wasn't just swearing in a few games, you were swearing at a level that hurt everyone's morale in a fair and semi-consistent string of different games.
You don't end up on the tribunal if you weren't reported on multiple occasions within a moderate period.
Instead of outing yourself, why don't you go send a message to Riot's support team like the pop-up suggests?
http://support.leagueoflegends.com - They take a few business days to get back to you, but I'm sure it would be more helpful to you then complaining on a fansite.

I act the same way with people (if not worse) I do on the forums and I haven't gotten penalized yet :T

Reiterating. You said you were reported three times. Does that mean 3 people reported you? Or was it 3 games? Or did you receive a temp. ban 3 times prior to this one?

This. I got banned for a ******** reason once before, but even though the reason was a load of ****... still my fault for raging.
I'd probably be permabanned by now if I played SR, but I only play Dom these days and I just can't get very angry over a game of Dom.
