Great first item, at that point health is very efficient to stop the enemy laner from destroying you and since I lately bring armor/lvl seals and mr/lvl glyphs to the lane, it lets my potential explode with the item. Also, as I already said, that shield basically autowins you the lane if used right.

Sterak's Gage seems rather meh as first item (at least from a theorycraft perspective) because everything except the flat health scales with levels and/or other items. As a first item, you won't get as much power/stats as you would compared to other items, not counting the passive. And hulking out can probably turn an all-in pretty easy, but I stil feel it would be most useful as a 2nd item. As a second item you'd have enough levels/base AD and health to really abuse the SMASH! time.

But
Sterak's Gage also scales negatively with levels as it pops easier the earlier you get it. You maybe get less out of it early, but early, flat health in general is stronger than later.
