Why play Thresh?

Pros


Very strong early
Good engage and disengage tools
You will have a lot of agency
Looks very flashy

Cons


Very skill dependant
Will struggle with weak adc
Runes
As I've already covered the keystones this will focus on the lesser runes.

Resolve


Tier 1:

Font of Life gives your adc slightly more sustain, but if you're playing with a lane partner that can't take advantage of it then taking Demolish will make it easier to take plates.

Tier 2:

Bone Plating reduces damage taken in a quick trade, but against a botlane that can easily proc it on you without putting themselves in too much danger then Second Wind gives you more regen against the poke matchups.

Tier 3:

Unflinching gives a good amount of tenacity, but if ever against a team without cc then Overgrowth gives a significant amount of bonus health.

If taken as secondary Bone Plating/ Second Wind and Unflinching/ Overgrowth are the best options.


Inspiration


Tier 1:

Hextech Flashtraption can catch your enemy off guard and lead to more picks.

Tier 2:

Biscuit Delivery will keep you healthier in the early game which is when Thresh wants to fight.

Tier 3:

Cosmic Insight will give your summoner spells more haste which will enable more aggression, but also gives item haste in case your running Locket of the Iron Solari or Redemption.

If taken as secondary Biscuit Delivery and Cosmic Insight are the best options.


In general Haste, Adaptive force and Armor are the best rune shards in my opinion, but you'll need to adapt to your enemies. You might need more magic resist or even more armor. If you'd like you can also choose more Adaptive force instead of Haste.
Items
As I've already covered the core itemization this will focus on what comes after.

Knight's Vow

is tailor-made for Thresh, it gives you great stats and helps you peel for your adc.

Zeke's Convergence

is similar to Knight's Vow as it also gives great stats, but instead deals more damage to the enemy instead of keeping your adc alive.

Redemption

is a great teamfight item, it helps in keeping your entire team alive during a fight. Don't be afraid of using it before a fight if you're about to take an objective.

Abyssal Mask

gives you a lot of magic resist and health for its cost. It also enables your ap damage dealers to do more damage.

Anathema's Chains

if the enemy has one super strong member and four bots/tanks then this will help your survivability against them. Also reduces their tenacity.

Mikael's Blessing

gives you more oppurtunities to peel your team if the enemy has some hard hitting cc spells i.e Enchanted Crystal Arrow or Dark Binding

Vigilant Wardstone

should always be your final item if you get this far. It gives you more wards on the map, more control wards in your inventory and some nice bonus stats as well.
Abilities
Generally starting with Flay is best as it gives you more power and allows for both proactive trades and defensive peel, however when playing against a double ranged botlane I like starting Death Sentence as it gives me the opportunity to catch the enemy off guard level 1.

If you're playing against a short range botlane you can take one or two more points into Flay and combining that with Steel Shoulderguards and maybe an extra Ad rune shard instead of haste your AA will do a lot of damage.

Later on when you've finished maxing your Death Sentence you should max your Dark Passage second because it just gives so much more value than Flay when you've left laning phase and entering teamfights.
