Early game poking will help tip things in your favor. When she dashes to try to proc her passive, I'd recommend combo-ing her.

The Secret to 2v1 double kills
The probably obvious secret to winning all top side ganks is in your passive and your ultimate. Your passive (Hemorrhage) bleeds opponents as well as adding high attack damage with max stacks. Guillotine(R) does more damage the more stacks of (p) you have.
Oftentimes, you'll feel a difference in aggression when a gank is coming. (The toplaner uses a crowd control ability seemingly randomly, or chasing for no apparent reason).
Vision wins games, so if you have constant vision of your brush, you'll know exactly where the jungle/midlaner/etc is ganking from.
Knowing when to initiate is key. If you have long enough where you can combo against your toplaner and max stack your passive, more times than not, you'll win. Even against a tanky top. The reason is, your stacks can pass to other champions for a short time after achieving max stacks. Oftentimes, the jungle will be more "squishy", so with max stacks, plus two abilities, you can finish off the jungle without much health being taken from you after ulting. Then turn on your toplaner and finish him, as a correctly timed ult can but used to finish multiple opponents.

CS-ing
Use (w) to cs early game, and throughout the game. I recommend pushing early, because the level advantage almost guarantees early game success. Levels 2 and 3 allow early game skirmish success. Most of the time, whichever gets to 6 first wins fights, especially with ganks.
When to Teleport
The only times your should teleport back to your lane; your first time back to lane or when you are about to lose your turret.
Oftentimes your bottom lane will have deep wards. Teleport to these when the enemy bot lane is pushed far enough. Even if you do not get kills, oftentimes flashes will have to be used to avoid death.
